Our Multi-Disciplinary Approach to Sciatica
Sciatica is not a single problem. The name describes a symptom, pain, numbness, or tingling running from the low back through the buttock and down the leg, but several different things can produce that symptom. A herniated disc pressing on the nerve root. A joint in the lower spine that has locked up and is creating pressure. A tight piriformis muscle in the buttock compressing the nerve from the outside. Often more than one of these is happening at the same time, which is part of why sciatica that gets only one kind of treatment often improves partially and then stalls.
At our office in Mooresville we look at sciatica from several angles at once, because the condition usually calls for it. The services we most commonly bring together for sciatica are chiropractic, spinal decompression, acupuncture, and dry needling.
The services involved
Chiropractic care. The joints of the lower spine are often restricted or misaligned in people with sciatica, which increases the mechanical stress on the disc and the nerve root. Adjusting restores normal motion to those joints, takes pressure off the surrounding structures, and allows the body to start recovering. For active sciatica we use low-force and instrument-based techniques rather than heavy manual adjustment, which keeps the treatment comfortable and avoids adding irritation to an already inflamed area.
Spinal decompression. When a disc is the source of the nerve compression, decompression is often the most direct tool available. The table gently stretches the spine to create negative pressure inside the disc, which draws the herniated material back toward center and reduces the pressure on the nerve root. Most people with disc-related sciatica notice that the leg pain begins to ease before the low back pain does, which is the clearest sign that the nerve is getting relief. A typical course runs several weeks, and we track progress closely throughout.
Acupuncture. The nerve pain in sciatica has an inflammatory component that mechanical treatment alone does not fully address. Acupuncture calms the nervous system, reduces the inflammatory response around the nerve root, and eases the muscle tension that builds up around the compressed area as the body tries to guard it. Many patients find that combining acupuncture with chiropractic and decompression produces faster and more complete relief than either approach alone. Dry needling. The piriformis muscle and the surrounding muscles of the buttock and low back often develop tight trigger points in response to the nerve irritation. These can persist and refer pain even after the disc or joint problem has improved, which is one reason sciatica sometimes lingers. Dry needling releases those trigger points directly, addressing the muscular layer of the problem that the other treatments do not reach as specifically.
The combined picture
Each service addresses a different layer of what sciatica involves: joint mechanics, disc pressure, nerve inflammation, and the muscular trigger points that develop around the problem. When we bring them together the care covers the full picture rather than one piece of it, and the results tend to reflect that.
We build the plan around what your specific examination shows. Not everyone with sciatica needs all four services. Some respond well to chiropractic and decompression alone, while others have a strong piriformis component and get significant relief from dry needling early. The exam tells us where to start and the response to treatment tells us where to go next.
Common questions
Do I need an MRI before starting care?
No. Most sciatica does not require imaging to begin treatment. If we examine you and find something that warrants a closer look, we will tell you and discuss next steps including referral for imaging if appropriate.
How long does sciatica take to resolve with this approach?
It depends on how long it has been present and what is driving it. Leg pain often improves before low back pain as the nerve decompresses. A disc-related case typically takes several weeks of consistent care. We give you a realistic picture at the first visit.
Is this instead of medication or alongside it?
Alongside it. If you are taking anti-inflammatory medication for sciatica, we work with that, not against it. Any changes to your medication stay between you and your prescribing doctor.
Do you offer a discount for combining services?
